  • Primary Member

    Our standard membership provides all the essential services for routine and preventive care.

    This tier is ideal for individuals and families seeking a dedicated primary care provider for their ongoing health needs.

    Included Services:

    — Unlimited in-person and virtual visits
    — Annual physical exam and wellness planning
    — Preventive care and screenings
    — Management of chronic conditions (e.g., hypertension, diabetes)
    — Sick visits and minor injury care
    — Direct communication with your physician
    — Coordination of specialty care

  • Primary Premium Member

    Our Primary Premium membership offers enhanced services for those who desire a more extensive and proactive approach to their health.

    This tier includes all the benefits of the Primary Member level, plus additional services designed for a comprehensive wellness experience.

    — 16-21: $99
    — 22-49: $149
    — 50-64: $159
    — 65+: $169

    Included Services:

    — All services from the Primary Member Level, plus:
    — Comprehensive annual executive physical
    — Advanced lab work and diagnostic testing (specific tests may incur additional, discounted fees)
    — Expedited scheduling, extended visit times, weekend and after hour access
    — On-site or at-home phlebotomy (blood draw) services

  • For Non-Members

    For individuals not enrolled in our membership program, we offer a selection of a la carte consultation services. Please note that these services are priced per visit and do not include the benefits of our membership tiers.

    Initial Consultation: $200
    — A comprehensive initial visit to discuss your health history, concerns, and goals. This is a prerequisite for all other non-member services.

    Medication Management: $150 per visit
    — Follow-up visits for the ongoing management and adjustment of medications for chronic conditions.

    ADHD Management: $175 per visit
    — Specialized consultation and management for individuals with Attention-Deficit/Hyperactivity Disorder.

    Medical Marijuana Certification/Follow-up: $200
    — Consultation and certification for medical marijuana use, as well as follow-up appointments as required by law.

Enrollment

A one-time enrollment fee is required for all new patients to cover the initial setup of your patient record and to ensure seamless integration into our clinic's system.
